  • Service Documentation: Dates, times, and types of services provided, along with notes from our team members related to service delivery.
  • Billing/Payment Information (if applicable): Financial details necessary for processing payments.
  • Website Data: If you use our website, we may automatically collect non-personal data like IP addresses, browser type, and usage patterns to improve our online service.

  • Directly from You or Your Representative: Information you provide during initial intake, assessment, and throughout the period of service.
  • From Service Documentation: Information generated by our team members during the course of providing and documenting your care.

Your information is used solely to facilitate the provision of your non-medical home care and for essential business operations:

  • To Provide and Coordinate Care: To create and manage your personalized care plan, schedule services, and communicate with your family/representatives.
  • For Payment and Billing: To process payments, manage accounts, and handle claims (if applicable).
  • For Agency Operations: To ensure quality of service, conduct internal audits, train staff, maintain records, and ensure regulatory compliance as a licensed Illinois home-service agency.

We may disclose Personal Information to third parties for business purposes as follows. However, we do not share phone numbers or SMS consent with third parties for their marketing purposes without your consent.
